Application Process

Tours

To arrange a tour and receive additional information, please click the button above and complete the inquiry form.

Once we receive your inquiry, we will reach out to schedule a school tour and provide an informational session. Please anticipate a processing period of 3-5 business days for us to address your inquiry thoroughly.

Acceptance

Admission is contingent upon Evergreen's capacity to accommodate your child's requirements, as assessed during the application process, and the number of available spots in each classroom.

Enrollment

Upon receipt of the acceptance letter, each family is allotted a seven-day period to finalize and submit the enrollment package.

REQUIRED DOCUMENTS

Prior to attending school, it is mandatory to complete all required forms. These documents can be submitted via Five Star Registration System portal, or alternatively, they can be mailed, emailed, or delivered in person to the school within the designated business hours.

Enrollment Forms

• Enrollment Agreement - Completed after acceptance.

• FARE Form

• Health Statement - Please print and have your child's physician sign.

• Immunization Records - Please submit an updated copy from your child’s physician.

• Allergy Emergency Plan - If your child has a diagnosed food allergy, please provide a written Allergy Emergency Plan from your child's physician.

• We welcome elementary-aged children for after-school programs and camps. Space is limited, so please reach out to us to check for availability.

Registration Fee

$100 non-refundable
$150 if there is more than one child

Family Referral Tuition Incentive

We are immensely grateful when you refer others to us, as it reflects the positive impact our school has on your child, and we wish to express our appreciation to you.

Spread the word about our school to your family, friends, and colleagues, and you'll get a $50 credit on your tuition once they've been enrolled for 90 days. Please note, this offer doesn't include siblings from the same family.

You can earn an unlimited number of referral rewards. Please ensure that your referrals mention your name when they contact our school, so we can properly credit your account accordingly.
